How to sign up for the Medicare Prescription Payment Plan

The first refill of the year rang up at hundreds of dollars because the deductible reset, and the pharmacist mentioned a payment plan. The wrong assumption is that Medicare enrolls you in it, or that you ask for it at the counter. You ask your drug plan, not Medicare and not the pharmacy, and the request has to be in before the expensive fill if you want that fill on the bill. Open enrollment for 2027 coverage starts October 15, 2026, and that is the moment to line this up so it is active on January 1.

Confirm you have drug coverage that qualifies

Anyone with a stand-alone Medicare drug plan or a Medicare Advantage plan with drug coverage can use the Medicare Prescription Payment Plan. Every plan offers it, joining is free, and it does not change your premium or your drugs. It only changes who bills you: the plan sends a monthly statement instead of the pharmacy collecting at pickup. It does not lower what you owe.

Check that it will help before you ask for it

Medicare's own before you sign up page says the option pays off when your drug costs land early in the year, and is a poor fit when costs are low or steady each month, when you already get Extra Help, a Medicare Savings Program, or a state pharmaceutical assistance program, or when you join after September, because there are too few months left to spread the bill. The yearly out-of-pocket cap for covered drugs is $2,100 in 2026 as of September 2026; your plan's Annual Notice of Change states the 2027 figure.

Ask your plan to enroll you

Call the member services number on your plan card or use the plan's website. Ask to participate in the Medicare Prescription Payment Plan; the plan reviews the request and mails a letter confirming you are in. Medicare's using this payment option page is the reference if the representative is unsure. You can ask at any point in the calendar year, and for 2027 you can ask once you have picked your 2027 plan so billing starts with the first fill in January.

Pay the monthly bill and keep the coverage

Each month the plan sends a statement showing what you owe, the due date, and how to pay. Miss a payment and you get a reminder; miss the date on the reminder and you are removed from the payment option, though your drug coverage itself continues. There is no interest and no late fee at any point.

If you switch plans for 2027. Participation ends when you leave a plan or move to a new one, so repeat the request with the new plan. Medicare.gov says participation renews automatically each year only when you stay in the same plan and do not opt out.
